Our team of licensed medical professionals such as Registered Nurses (RNs) or Licensed Practical Nurses (LPNs) offer skilled nursing services in your home.

Our Home Health Aides can assist you with activities such as bathing, dressing, or provide assistance as you move about your home.

Our Physical Therapy team is responsible for assessing, planning, participating and organizing rehabilitative programs that improve patient mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.
Examples of instances PT may be needed may include, but are not limited to the following:
-Ortho Related Injuries
-Knee/Total Hip Replacement
-Stroke/Cardiac Event (Heart attack)
-Parkinson’s Disease
-Arthritis/Degenerative Joint/Bone Disease*PT can be a standalone service.

Occupational Therapists treat injured, ill or disabled patients through the therapeutic use of everyday activities. They assist the patient in development, recovery, improvement, as well as maintain the skills needed for daily living and working.
Examples of instances when OT may be needed may include, but are not limited to :
-Decline in ability to dress/bath
-Inability to safely do household chores
-Recent shoulder surgery or exacerbation of previous injury -
Our Speech Therapy team works to assess, prevent, diagnose, and treat speech, language, social communication, cognitive-communication, and swallowing disorders in both children and adults.
Our team also offers LVST Loud and VitalStim Therapy.
LVST Loud is a therapy method that trains people with Parkinson’s Disease to improve their voice and speech intelligibility. It has been researched and clinically proven to improve voice volume and speech intelligibility.
VitalStim Therapy is designed to help swallowing through the use of Neuromuscular Electrical Stimulation (NMES). Electrical stimulation is used to aid muscle strengthening to rehabilitate the swallow. The sensory stimulation that is provided also helps muscle recruitment and swallowing function.*ST can be a standalone service.

Our team of Medical Social Workers are available to assist patients with accessing community resources, assessment of the social and emotional factors related to the patient illness, need for care, their response to treatment, and adjustment to care.
